Yes, you can achieve quite good results — not 100% perfect, since there are so very many factors that interplay here.
Your first obstacle: you say you know the CMYK mixture.
But how to convert that into RGB? Watch what color space you convert into: Color Management: Color Space Conversion
Is your monitor properly calibrated?
As I see it, you use that card as a reference, to be matched.
